OVERVIEW

A quality plan, supported by a strong personal network, can play a part in a person achieving the good things in life. Planning encourages us to think about the future, what we want to do and how we will get there. It’s important to keep in mind that a plan is one thing however following through on the plan is what makes life happen.

Family Lead Planning

Family members are in the best position to contribute to and be actively involved in the planning process when people are not able to lead their own plan. It’s families who understand what gives meaning to their family member’s life, what counts and what it takes for a person to flourish.   Family members and friends are encouraged to be full partners in planning.  We call this family-led planning

TOOLS

There are many planning tools that have stood the test of time.  We use whatever fits the current situation for the person who is at the center of the plan. A plan can be simple or more detailed, it really depends on each individual and their circumstances.  

Individual Service Design – Is an intensive planning process that enables people to think about their support and design the best kind of support arrangements.  An Individual Service Design can be effective when people are thinking long-term, and who will need to safeguard those arrangements by building a support system around them (this can be paid and unpaid).

PATH - Planning Alternative Tomorrows with Hope. “Go forward with your dreams. Live the life you have imagined.” (Thoreau) A PATH is a facilitated planning process designed for people to help them realise their dreams, aspirations and goals for a life that is in line with their values.  The PATH plan can be useful for people who want a visual illustration of where they are now and where they want to be in the future. 

MAP - Making Action Plans is a planning process for people that begins with getting to know the person who is at the center of the plan.  MAP can be used if someone has a problem they want to sort out, or when they have a particular goal they want to achieve. People invited to a MAP planning session are invited to identify a person’s gifts, strengths and skills and how these personal attributes can be used to the person’s best advantage.

Outcomes

Regardless of the planning tool used, we will provide you with a summary of where you are now, with well-defined steps to help you get to where you want to be.

Cost

The cost really depends on what planning tool you want as the approach can range in the time it takes to plan and therefore informs the price, so gives us a call or send an email to discuss your options.

Living a life with purpose and meaning is at the heart of the matter. Choice and control doesn’t mean you’re on your own baby!

Whanau, family and family culture all have a bearing how family members thrive.  

Making Informed decisions includes:

  • making reciprocal connections that matter

  • living in communities where our personal values and culture is respected

  • creating supportive environments

  • assessing reasonable risks- as we know, there are trade-offs in life

  • learning how to make decisions for myself, and what’s involved in supporting others to make decisions and maintain control over those decisions.  

 Knowing where you’re going includes making decisions and/or supporting choices about:  

  • Where you live

  • Who you live with

  • What you do with your time

  • What you do with your resources

  • Who’s around to support the choices you make

And if you choose to live a typical life with others in the community of your choice, what happens when your family or people in your networks are no longer able to be involved, or have ‘passed on’?  This situation can be counted on for sure and is a life-event we all need to plan for. It’s worth being open to having conversations with other people about what they’re doing to manage arrangements that really matter, especially when a family member needs other people involved.

Family Network NZ invites you to have a conversation with us about people who only a decade ago were thought best placed in residential services, who less than 30 years ago were institutionalized. We’ll talk about people who are in paid work, who own micro-businesses, who volunteer their time to community. who rent with flatmates, and we’ll tell you about people who enjoy their own company who are making good lives happen for themselves- using the resources available to them.

We won’t lie to you or gloss over the hard bits. At the heart of every person’s story is Family.
